Adult Circumcision with Stapler Device (ZSR/CircCurer)

Adult circumcision with CircCurer stapler is a quick procedure. Compared with conventional circumcision with stitches, stapler circumcision is associated with:

  • Shorter surgical time
  • Higher patient satisfaction with penile appearance
  • Lower operative pain
  • Less operative blood loss
  • Lower wound bleeding post-operatively

However, wound healing takes longer than traditional circumcision in adults.

Anaesthesia

A local anaesthetic is used for stapler circumcision, injected into the base and around the shaft of the penis. This provides safe and effective pain relief during the procedure and minimises post-operative pain for a few hours.

Informed Consent

Patients are provided with all information about stapler circumcision ahead of time.

Stapler circumcision is irreversible and permanently exposes the head of the penis. Informed consent is taken before the procedure.

Full or Partial Circumcision

  • Full circumcision: moderately cutting inner and outer foreskin to fully expose the head of the penis.
  • Partial circumcision: leaves loose skin which partially covers the glans.

Partial circumcision may not work in all adults. A full circumcision may appear like partial circumcision in men with buried penis. Partial circumcision is possible but can have long-term complications.

Procedure Technique

For adults and boys over 16 years:

  1. Administer local anaesthesia.
  2. Retract foreskin and assess suitability.
  3. Stapler circumcision not advisable if there is scarring due to BXO.
  4. Apply the bell of the instrument inside the foreskin.
  5. Secure correct position of the stapling device.
  6. Cut the skin and apply staples simultaneously.
  7. Apply a bandage around the wound at the end.

Recovery

Stapler circumcision heals quicker than stitches.

  • Avoid swimming and weight lifting for 2–3 weeks.
  • Full recovery usually requires 4–6 weeks.
  • Over-sensitivity of the glans may last a few months.

Healing Tips

  • To reduce effects of night erections, empty bladder before sleep and during the night.
  • Sleep on your side or back, knees slightly raised.
  • Wear tight underwear to minimise movement.
  • Keep wound dry for five days.
  • Take vitamin C for 10 days to promote healing.
  • After staples are removed, apply vitamin E cream, Mederma gel, or bio-oil to improve scarring.

Medical Evidence

Five recent studies involving 2026 men compared stapler with conventional circumcision:

  • Less intra-operative blood loss
  • Shorter operating time
  • Lower mean pain scores (day of surgery and post-op days)
  • Faster wound healing

Note: 88% of patients required removal of residual staples at 4 weeks.

Risks of Stapler Circumcision

Patients are informed of possible risks:

  • Failed staple procedure requiring traditional circumcision
  • Bleeding, bruises, haematoma
  • Infection
  • Retained staples >4 weeks
  • Unintentional damage to the glans
  • Removal of too much or too little skin
  • Lymphoedema
  • Poor cosmetic results
  • Changes in sexual sensation

Post-operative erections may cause pain, bleeding or discomfort, and sometimes disruption of staples or scarring requiring revision.

Postoperative Care

  • Discomfort for a few days (painkillers help).
  • Passing urine usually unaffected.
  • Swelling and bruising of the shaft for 2–3 weeks is normal.
  • Bleeding can be controlled with pressure or a bandage.
  • Dressing (Cobain) applied for 3 days to protect wound.
  • Healing requires 2–3 weeks; full recovery 4–6 weeks.
  • Avoid gym/weights for 2 weeks; sexual activity for 4–6 weeks.

Most patients require 2–3 days off work. Showering is safe after one week if wound is dry.

Follow-Up Visit

Some staples fall off themselves, but residual staples often remain after 3 weeks.

All patients require a follow-up appointment at 3–4 weeks for staple removal.
