Preputioplasty is a foreskin-preserving operation that widens a tight foreskin so it can retract more comfortably, without removing the foreskin. It is most often considered when you have phimosis (a tight foreskin), and you want to keep the foreskin.
Preputioplasty is usually not suitable if the foreskin is scarred or inelastic due to BXO / lichen sclerosus, because the tightness is more likely to recur, and symptom relief is less predictable.

Who This Is For
Preputioplasty may be suitable if:
- Your main issue is a tight foreskin that is difficult or painful to retract, especially during erections
- You want to retain your foreskin
- There is no suspicion of scarring disease affecting the foreskin on examination
It may be less suitable if:
- The foreskin is very tight and scarred, particularly with suspected BXO/lichen sclerosus
- You have recurrent infections and scarring that make a surgical solution more appropriate
When It Is Recommended
We may recommend preputioplasty when:
- conservative options (such as careful hygiene, stretching or topical steroid treatment) are unlikely to treat your symptoms or have not helped enough
- You want a surgical solution, but prefer to avoid full foreskin removal
- Your examination suggests the tight ring can be widened safely and reliably
Alternatives
Depending on your symptoms and findings, alternatives may include:
- Observation / topical treatment: if symptoms are mild and there is no significant scarring
- Full circumcision: removes the foreskin and is often the most definitive option when there is significant scarring or recurrence risk
- Dorsal slit: a slit in the tight band to relieve tightness, usually reserved for specific situations where a quick decompression is needed, or circumcision is not suitable
- Frenuloplasty: if the main problem is a tight frenulum rather than the foreskin itself
Techniques And Options We Offer
“Preputioplasty” describes a group of techniques that widen the tight part of the foreskin without removing it.
A common principle is to make one or more planned incisions through the tight ring and then close them to widen the opening.
Depending on your anatomy and the pattern of tightness, the approach may be described as:
- Limited incisions to widen the tight ring
- V-Y style closure in selected cases (often discussed for older boys more than 16 and adults who want foreskin preservation)

Anaesthesia And Comfort
Preputioplasty may be performed under local anaesthetic in suitable adults, meaning you are awake, but the area is numbed.
Some services perform foreskin-preserving surgery under general anaesthesia in hospital settings.

What Happens On The Day
Assessment and consent
- We confirm the diagnosis (tight foreskin vs scarring disease).
- We discuss your aim: foreskin preservation vs the most definitive option.
- We explain realistic outcomes, including the chance of recurrent tightness.
- We confirm the plan and obtain informed consent.
The procedure
- Local anaesthetic is given.
- The tight ring is widened using planned incisions.
- The skin is re-stitched to create a wider opening, using dissolvable stitches and medical tissue glue.
- A light dressing may be applied.
- You go home the same day with aftercare instructions by email.
Recovery And Healing Timeline
Most men can go home the same day.
What you can usually expect:
- In the first few days, swelling and bruising can occur.
- 1–2 weeks: the wound edge settles; dissolvable stitches may still be present.
- Several months: scar tissue continues to mature and soften.
Time off and activities:
- Work: many people take a few days off (or longer, 1-3 weeks if their job is physically demanding).
- Exercise and swimming: typically avoided for 2 weeks until the wound is dry and healing well.
- Sex and masturbation: usually avoided for around 4–6 weeks, or until healing is secure.
Risks And Complications
As with any surgical procedure, risks can include:
- bruising and swelling
- bleeding
- infection
- scar tenderness
- cosmetic dissatisfaction
Specific considerations with preputioplasty:
- The foreskin can tighten again during healing or later due to scar contraction, and some men may ultimately need circumcision if symptoms recur or do not improve enough
- The foreskin’s shape may look different afterwards (some men describe a “fish-mouth” appearance)
Aftercare And Follow-Up
Aftercare aims to support healing and reduce the risk of the foreskin tightening again.
Do
- Keep the area clean and dry as instructed.
- Rinse gently after using the bathroom, if needed, and pat dry.
- Wear supportive underwear to reduce movement and friction.
- Use simple pain relief if suitable for you (for example, paracetamol/ibuprofen).
- Once advised it is safe, retract the foreskin gently and regularly during healing to prevent re-tightening (we will tell you when to start and how).
Don’t
- Don’t force retraction if it is painful in case the wound is not ready.
- Don’t soak the area (baths/swimming) until the wound is dry and healing well.
- Don’t restart sex or strenuous exercise until the wound edge is strong and comfortable for 6-8 weeks.
When To Seek Urgent Help
Seek urgent medical advice if you have:
- Bleeding that does not settle with gentle pressure
- Increasing redness, heat, swelling, worsening pain, or pus-like discharge
- Fever or feeling generally unwell
- Difficulty passing urine or inability to pass urine
- The wound opens or sudden worsening pain after a pull/tear
